You will need the following documentation:
1. Completed application, OWF77-1 (E) Be sure it is signed by your commander.
2. A copy of your current CAP card
3. A copy of your current Ohio driver’s license
4. An abstract from the BMV
5. Write your email address, somewhere on the front of the application.
