The CAPF 15 Application for Cadet Membership has undergone some revision. The previous form included a page for measurments for for the free cadet uniform. The revision does not include this page. Instead, when the cadet receives his/her CAPID number, they log into eServices, click on the Cadet Uniform link, and inputs their size for their blues. If you still have some of the previous CAPF 15's, just remove the uniform page. Only the front and back of the Form 15 is sent in to NHQ now.
